a Disktop programme for creating a PDF certificate

I need desktop software to create PDF certificates with text, images, and unique codes. It should also be able to store the certificates and marks obtained. The software should ask for the person's name, gender, and the number of lectures attended using a form, and keep it in a database for later use. All these features together will help me create certificates quickly and safely.
