Background check statement

Need a statement explaining my background check red flags. I have a DUI misdemeanor and due to that I ended up in non-disciplinary recovery program for nurses. I've been compliant for a year now. Just need help writing a statement that basically states that I'm a good candidate regardless of my dui. I've been working hard, I've learned a lot, and I accept my mistakes. I worked for this hospital before and I was always a great employee and would like a chance to be part of it again.
