I need help reviewing and completing a custom embedded controller schematic and PCB based on an ATmega328P-PU.

5 V, 3.3V operation
Power
12 V DC input
On-board 5 V regulator for the MCU
12 V rail available for loads
Proper power distribution and decoupling
Inputs
The controller monitors six analog/digital sensing lines.
Input assignments:
Input A0
Input A1
Input A2
Input A3
Input A4
Input A5
These inputs are used for continuous signal monitoring.
Outputs
The controller drives multiple indicator groups.
Required outputs:
Output Group 1
Output Group 2
Output Group 3
Output Group 4
Output Group 5
Output Group 6
Each output controls multiple LEDs simultaneously through a transistor driver.
The controller also drives:
One 12 V buzzer
