Digitize Paper Form Entries

I have a stack of paper forms that need to be converted into a smooth, searchable workflow. What I need is an online form—Google Forms works perfectly—that mirrors every field on the original documents and funnels each submission into a live spreadsheet.

The finished form must:
• Accept file uploads so respondents can attach scans or photos.
• Trigger automated email notifications—one confirmation sent to the person filling it out and one alert sent to me.
• Enforce solid data-validation rules to stop incomplete or badly formatted entries.
