Repair Turbify PHP Email Form

The form originally worked and successfully sent an email. I can provide all the files for the freelancer to revise.
The website is hosted by Turbify and uses PHP.
Problems began after changes were made to add unsubscribe-related email headers. Since then, different revisions have caused intermittent delivery, redirect problems, MIME/Base64 formatting issues, and conflicts with Turbify/Imunify360.
Required form behavior
The form should:
Collect email address only—no name field.
Display as a compact, responsive box within the existing page.
Send the prospect a Texas startup package summary.
Send a notification to info@xxx.
Send from:
Allow the prospect to reply directly to info@xxx
Redirect successful submissions to: specified webpage
Display a clear error if delivery fails.
Work repeatedly with different email addresses.
Work with Yahoo/Ymail, Gmail, Outlook, and other common providers.
Avoid duplicate submissions and obvious bot abuse without blocking legitimate Safari autofill users.
