3 phase power monitor

Need to monitor incoming 3 phase power (230, 380 & 480v). Current draw is 10A max to drive an attached motor. Monitor incoming power for: 1) Phase reversal, 2) Phase loss, 3) phase brownout (variable 2% to 15% trimmer adjust), 4) phase frequency shift (60Hz/50Hz standard.... more than 10% deviation needs to trip system - trimmer selectable). LEDs to indicate each of the 3 phases have power. LED to indicate proper phase sequence. Must be designed without requiring a programmable micro (ASIC ok). Must be designed with thru hole components. UL or UR 105 deg C components. Milestones: Schematic with bom (25%), after prototype is built and schematic is proven (75%).
