Design Circuit Board / PCB Power Distribution board.

Hi,
I'm wanting a circuit board / pcb created.
Input volts is 12v. (standard size female input module)
Two lots of power distribution voltage.
6 x 12v.
4 x 6v.
Input power should have a fuse holder 3AWG or compatible before it goes to the rest of the outputs.
There should be a small capacitor between the input and the outputs to keep load steady.
