ZenDesk Form Design and Workflow

I am looking for a freelancer to design and customize ZenDesk IT support forms for our company, we are testing ZenDesk as a potential ITSM platform. The primary purpose of the forms/recipes is to collect customer inquiries, incidents, requests, and work orders. Although I have a general idea of the fields and information I want to collect, I am open to suggestions and recommendations. I do not require any specific integrations or automation with the forms, just basic functionality. If the platform leads to production, then there may be more work with automation and macros. Ideal skills and experience for this job include:

- Experience with ZenDesk form customization
- Strong understanding of customer inquiry, ZenDesk workflow and ITSM platforms
- Ability to create a visually appealing and user-friendly form design
- Excellent communication skills to ensure the form meets my specific requirements and needs.
-Time Zone will be similar to ET USA, preferably from 7:00 AM to 7:00 PM
