Prepare Word Docs for DocuSign -- 2

We have several documents in Word format that we want set up as a form in DocuSign. Basically want to set it up so our staff can brainlessly enter the required information in offer letters and some other docs and have everything filled in in the appropriate space. Attached is an example of one of those docs and the one we would begin with.
