PDF to JotForm Conversion

I have a Fillable PDF Reference Form that needs to be converted into a JotForm. This form will be used for gathering reference information for our company.

Key Requirements:
- Preserve the Text fields and Checkboxes: The original PDF Reference Form includes various text fields and checkboxes. These elements are essential for capturing the necessary data from our references.
- Design Elements: The JotForm version of the reference form should not include a logo heading. However, it should maintain the same question/answer structure and text as the PDF form.
- Functionality: There are no specific calculations or file upload functions required for this form. It should be simple and straightforward, focusing on collecting the reference data accurately and efficiently.

Ideal candidates for this project should have:
- Experience with JotForm or similar form building platforms
