One page Fillable form that accepts electronic signature from an epad

One page form. I have the mockup already done in LibreOffice. But I need the form to “link up” with my epad to collect two signatures before printing. I have a drug testing company and want a fillable form to complete rapid drug tests. The form needs to reset after printing and would ideally record the details captured in some sort of database or other record
