Microsoft Forms where data is automatically logged into a table

I am looking for a freelancer who can create a Microsoft Forms project for me. The project involves creating a form where users can input data and have it automatically logged into a table. Here are the project requirements:

Data Collection:
- The form will collect data using multiple choice questions, drop down menu and open answers

Table Format:
- I do not have a specific format or template for the table where the data will be logged.
- I am open to any format and design suggestions.

Form Fields:
- The form will include 6-10 fields for users to input data.
