Manage incoming results from a MS Forms survey to c500 clients

We have c500 unique clients and an MS Forms survey (already drafted in our Office 365 tenant) that we wish to issue. As well as the aggregated results that MS Forms provides, we need to be able to allocate a unique identifier to each survey request and link the individual responses back to the relevant client record on our CRM, which is build on an old Visual Fox platform.
