Jotform document to be created for mobile phone/tablet/ipad/computer friendly
Budget: $30 – $250 AUD
Currently have a word document with 10 questions that need to be completed by our students prior to enrolling into our training courses.
The word document needs to be re-created in Jotform so it is an editable document that is mobile phone/tablet/ipad/computer friendly, so students can just complete the form on their mobile via link sent by myself. It then needs to be saved as a PDF by the student so they can email it back to me. The form is a combination of written answers and multiple choice tick boxes.
