Jotform Form and Report Template for Child Psychology Practice

We are looking for someone to assist us with streamlining the patient background information we collect at our child psychology practice. We are wanting the background information collected from parents via an electronic form to auto-populate a report to which we will add other information at a later stage.

The project will involve:
- creating a form using JotForm (or other recommended platform that has capacity to auto-populate a report and has the option for secure data storage for our confidential information) from a list of questions to be supplied in Word or PDF
- create a report template in word or google docs (preferred) that is auto-populated by the responses that our patients have entered into the JotForm
- Providing a clear method for using the form and template that our team of psychologists can easily follow
