I am looking for a freelancer who can create a fillable questionnaire in either Word or PDF format. The main requirement for the project is that the answer portion of the questionnaire is very user-friendly.
Preferred File Format: The client has no preference for the file format and is open to either Word or PDF.
Design/Layout: While the client does not have a specific design in mind, they do have some ideas and are open to suggestions from the freelancer.
