Digital Intake Forms Creation

I'm looking for a freelancer who can help me convert my current paper intake forms into digital, online forms. The primary objective is to completely eliminate the use of paper forms.
There are a few more documents that I couldn't upload also, I have to print them and then scan them in but the majority is here. I need certain forms to be separated but we can talk about the details

Key Requirements:
- The submitted form data should be emailed directly to me for immediate access and response.
- The digital forms must include a feature for file upload. This is crucial for clients to attach any necessary documents.
- Incorporate an electronic signature feature. This will allow clients to sign the form digitally, making the process more convenient and efficient.

Ideal skills for this project:
- Proficiency in form creation platforms such as Google Forms, Typeform, or other similar software.
- Knowledge of how to implement file upload and electronic signature features in online forms.
- An understanding of data security and confidentiality to ensure the safety of the information collected.
