Customer order form with T&Cs

I would like someone to finish the order form I have made.
Presently, I have a preprinted pad where I tick the boxes of the work that will be completed then the customer signs to acknowledge the work, T&C's and OHS risks. I want to be able to do this on a tablet, the customer receives a copy, accounts receive a copy for billing. I have entered most of the info but am stuck on the modal box. Looking for someone who can help finish the project. I
