Create a multistage form in Divi (WordPress) and form management functionality
Budget: $750 – $1,500 AUD
I am looking for a freelancer to create a multistage form in Divi (WordPress) and implement form management functionality.
Purpose of the Form:
- The form is a confidentiality declaration being collected from a defined team of people.
Design/Layout:
- I have a simple design in mind for the form, but I am open to suggestions from the freelancer.
Number of Stages:
- The form should have 4-6 very simple stages for users to complete.
Ideal Skills and Experience:
- Proficiency in Divi (WordPress) to create and customize the multistage form.
- Experience with form management functionality to set up multiple projects and teams of people who need to complete the form. Reporting on status of form completion for each team member. Two factor authentication when form submitted by each team member. Forms data must stay on our website.
- Strong attention to detail to ensure the form functions smoothly and accurately.
- Ability to work collaboratively and provide suggestions for design improvements.
