Create Google Docs form

I want to create google docs form so that my employees can record their daily activities and follow a timetable when they are carrying out their work. We need a daily log of all activities, enabling employees to use either their voice to text or type to log their responses. The logging process must be simple whereby Employees are prompted to answer questions on a form but are able to respond using their voice to text or type. Once the form is complete I want employees to be able to share the form to sync directly so that any authorised employee can access the form from anywhere remotely. Once the.form is completed at the end of the day I want the proces to automatixally start a new blank form the next day. The process should recur daily until ended by the manager in our office.
