Correct Contact Forms and Create a mobile menu for website
Budget: $30 – $250 USD
I am having some issues with 2 forms on a wordpress website using contact 7
On Form 1
One question is a use/no responses but the form is sending the label name through via email as the answer and not the users answer (yes or no)
On Form 2
When the user omit answers to any questions that have a check box, and click the submit button, nothing happens, no error message to inform the user they have a missing field and the screen does not jump to the missing field. There are 2 questions with check boxes.
When the Telephone number field is not entered the form does not bring the user to that field to show them which field is missing
Forms with check boxes
On both forms, these 2 questions below are not being received in the email.
1. Have you been diagnosed with Schizophrenia, Bi-Polar disorder or Borderline Personality ?
2. Do you have suicidal thoughts? If so how often and how recently ……
Also I would like the main menu from www.awholenewhigh.com to appear on the phone version of the website, right now it does not.
