Automate Contact Form to Generate PDF and Email

I am looking for a freelancer who can help me automate my contact form to generate a PDF and send it via email. The project details are as follows:
Platform:
- My contact form is currently hosted on Jotform.
PDF Template:
- I have a specific PDF template that I would like to use for generating the PDF.
Email Service:
- I prefer to use Outlook/gmail as my email service for sending the generated PDF.
Skills and Experience:
- Experience in working with Jotform, creating PDF templates, and integrating with Outlook/gmail
- Strong understanding of email automation and the ability to set up automated workflows
- Attention to detail to ensure the generated PDFs are accurate and properly formatted.
- Excellent communication skills to collaborate with me and understand my specific requirements.
I will fill out my contact form (https://form.jotform.com/232953539389371) with the required data. Somehow, the attached files should be inserted into a PDF automatically. Once the PDF is automatically filled out, the PDF and the MP4 file should be automatically attached to an email and sent automatically.
