MS Word Operational Procedures Document Template

I want to create an MS Word document template which will serve as our common platform for operating instructions and standard operating procedures. Each specific procedure will be different, but the structure of the document will be the same (e.g., there will be 9 main headings on every document). Each document will have the following headings: 1. Purpose, 2. Revision and Approval, 3. Process Definition, 4. Process Objectives, 5. Process Owners and Responsible Parties, 6. Safety Considerations, 7. Procedural Steps, 8. Quality Control, and 9, Definitions and Acronyms.
The document will have a Header and a Footer; the header will have a common banner with the company logo on the left margin and a text block on the right margin (with company name, process title, date/year, revision number and author initials). The text block will have cells (fields) that are editable: process title, date/year of revision, and author initials. The footer will have only page numbers, centered horizontally.
The document will have only numbers for the individual headings and sub-headings. The basic format is left justified and each sub-heading will be indented to present a clean, organized look. These sub-headings are to extend out to 6 levels. Graphics and photos will not be numbered but will reside under the specific heading/sub-heading aligned to the specific level and will not interrupt the numerical sequence within the document.
This document is to be saved as a template so that each new instruction issued will have the same style. I have a draft document that is available upon request.
