Help fix format with Adobe or any type of PDF for clinical notes

We’ve been trying to create a template that will work for our clinical notes because we serve autistic kids. I spent an enormous amount amount of time trying to format, hire, edit and create something that’s purposeful and practical for our business but nothing has worked. I have a couple of nice forms that I like, but the implementation and use your friendliness. What is adobe is problematic.
