Formatting and edits to Word document

I am in need of a freelancer who can help with formatting and editing my Word document. Specifically, I need a document formatted the same as an existing document (provided) as well as a couple of minor edits. The document is around 35 pages in length. The ideal freelancer for this project would have experience with Word formatting, proofreading and grammar, and be able to work efficiently to meet deadlines.
