Formatting/editing of esoteric questionnaire

I am writing a book and part two is a narrative from individuals who answer a very specific questionnaire. Their answers from the questionnaire is how the different narratives get created. Ultimately, section 2 will be a compilation of these narratives to highlight one of the purposes of my book. This purpose being to showcase the similarities and differences of the awakening process to being. I need the questions 1) proof read and grammatically adjusted. I need the questions 2) formatted so that it looks professional and neat. I’m also into the order of the questions 3)being adjusted to help the flow but that is not mandatory. I am open to accepting that feedback if the right eye feels guided to. However, My main priorities are 1 and 2. 4) is the potential in collaborating with you further in you placing the answers into the narrative. The answers will not be manipulated in any way but formatted to create a picture of that individuals experience.
