Create Fillable PDF Form for Business

I'm in need of a professional who can create a fillable PDF form for our clients. The form should be user-friendly and designed with the utmost attention to detail.
I've created yes/no questions, select an answer, and/or fill out a field based on the question. This is a questionnaire for our older clients that don't like using online apps. This form will be emailed to them to fill out specific fields. The form will need to be locked so no one can edit the questions.
I have attached a document as an example with the questions. I wanted to share so you could get an idea of what I'm trying to accomplish.
