Add Internal Hyperlinks to Word Document

I need someone to modify my Word document book to add internal hyperlinks.

Requirements:
- Add hyperlinks to connect:
- Table of Contents
- Index
- Text chapters
- Hyperlinks should work within the same chapter and between different chapters

Ideal Skills & Experience:
- Proficient in Microsoft Word
- Experience with creating and managing internal hyperlinks in documents
