Payment Gateway integration for adult website

We are running a small adult website and we have substantial problems with our payment gateway at the moment. By "payment gateway" we mean a system which will allow our clients to pay via credit card.

We need to install a stable and reliable payment gateway which
a) accepts adult content and
b) deals with companies outside the US / EU, in our case, Brazil.
We pay per hour.
