Design clinical forms
Budget: $30 – $250 AUD
We are looking for a graphic designer to create professional and visually creative forms for use. This agreement is for ongoing work for the right designer. Bidders must be able to show the following:
- High proficiency with Adobe Creative suite products
- Graphic design qualification
- Excellent portfolio
- Experience with design to support health care providers
- Excellent communication and interpretation of scope
- Demonstrated ability to ask the right question.
We will provide one scope to the winning bidder. The performance during that scope will be used to determine if we will have an ongoing business relationship. The form fields we require are attached.
