Publish a Health care book on Amazon / Kindle

I have a manuscript for a book on workflows in healthcare. I would like to self-publish on Amazon / Kindle. I would also like to be able to order a few physical copies if needed. You will need to design the cover, and set the typography and format for electronic and print media.

The manuscript is done and is in word docx format.

I have some diagrams and tables in the book that would need to be formatted for the two layouts
