Help me format my ebook for sale on Kindle and Amazon!

Project is 40-ish pages long. I want it set up to read more like a poetry book, with a title and entry on each page. Some posts are longer and will require more than one page. I'm not sure how to set it up so I'm hoping for someone to help me with that along with creating the cover, title page, and copy right information. I will send over samples after being contacted.
