Ebook design and formatting

I have a basic 25-page PDF that I would like to convert into a proper ebook that I would then sell from my website. Once purchased people can choose which format to download - epub, Mobi, as well as PDF. The book consists of 100 expressions in Brazilian Portuguese - each PDF page has 4 expressions, and each expression has an explanation in English, an example of a sentence and a photo related to the expression. In the ebook, perhaps there would be two expressions on each page. (I've included a page of the PDF that I have). That would include the cover and a clickable table of contents.
