Convert Published Book to eBook via Production of ePub file

I am a Christian Pastor author, having published several books. Hence I want to convert one important volume, of 415 pp, to a reflowable eBook. A PDF copy is attached. It has three detailed indexes that must be included, and so far this has presented a problem. It concerns the fact that because of some slight text and layout editing, the MSS is now 453 pp. The indexes included here are from the original published edition with numbers that need to be adjusted to the updated text provided here. Can this, as well as the eBook conversion, be done at a reasonable cost?
