Drupal 8 Custom Module Work

I am in need of a developer that can create a custom module for DRUPAL 8. I have a Content-Type called "Policy" that I would like to add some Entity-References to another Content-Type called "Endorsements". When a user creates a new Endorsement, they check some boxes and fill in some fields that will then be used to place the Endorsement on the "Policy" - the catch is - I need more than one checkbox for each of these Endorsements when they are referenced on the Policy form - I need to show 2 checkboxes and a textbox with a default value from the Endorsement node.
