Build an Azure Website with a few sub-pages

We are to build a form that has 3 sub-categories. The data gathered in this form is entered by employees within Azure AD. Data should be stored in a database and be revisited and are subjected to change.

The questions of the form is aligned in the following manner, without enclosing the actual content:
What food do you like:
Fruite
-Apple
--From a scale 0-10 how much to you like it?
-Oranges
--From a scale 0-10 how much to you like it?
-Bananas
--From a scale 0-10 how much to you like it?
-Grapes
--From a scale 0-10 how much to you like it?

this should be split up to first asking the first question, next page is the sub categories, then third page is the score. This response should be stored on UPN name for the one that created it, the data should also be stored with the latest date this was updated, added or changed.

page 2: should include a page where you can search for these different foods, find matching UPN names and the score they rated for that food.

this needs to be tied together with Azure AD, built as a web app in azure utilizing azure SQL DB. estimated entry of data rows is approx. 10k.

The application needs to be built secure.

The design of the solution should be extremely simple.
