SQUIRE 2.0 Hospital Pharmacy Standards

I’m putting together a SQUIRE 2.0-aligned reporting standard designed specifically for our hospital’s refill pharmacy. The finished document needs to keep Patient Safety and Efficiency Improvement front-and-centre so our pharmacy staff can quickly capture and share quality-improvement work that raises patient satisfaction, trims medication errors, and speeds up refill processing times.
What I’m after is a practical, ready-to-use package—not an academic treatise. The standard should mirror the familiar SQUIRE 2.0 structure (Title/Abstract, Introduction, Methods, Results, Discussion, etc.), but every heading and example must relate to day-to-day refill activities such as prescription review, dispensing checks, and hand-off communication. Plain language, pharmacy-specific examples, and clear data-collection prompts are essential so technicians, pharmacists, and residents can all fill it out confidently without extra training.
Deliverables
• Editable master guideline (Word or Google Doc) mapped to each SQUIRE 2.0 item
• One-page quick-reference sheet for busy counters
• Implementation checklist that managers can tick off during rollout
• Final PDF formatted for hospital intranet posting
Acceptance will be based on an internal pilot: three frontline pharmacists should be able to complete a mock report in under 10 minutes using your template with no outside help. If that criterion is met—and the content accurately spotlights patient-safety and efficiency metrics—the project is done.
