Need Guest Registration Form Built

I have the form already created as a PDF and is done. What i need help with is finding a way to TEXT this form with the capability of filling out each box.
The boxes will be just typed in with Name, age and phone number.
This form will need to be capable of being sent in text format and filled out from a mobile phone.
The form needs to be protected so it can not be altered.
