I work for a fast-paced Physical Medicine & Rehabilitation practice in the East Bay and I’m looking for an experienced medical scribe remote Monday through Thursday. Your primary focus will be getting every encounter documented accurately and promptly so I can stay fully present with patients.
You’ll handle all follow-up notes, create complete new-patient charts, and keep progress notes current. That means documentation in real time and after visit consultations, structuring the history and exam the way I dictate, and pulling any prior information from the EHR that belongs in today’s note. By day’s end I expect each chart to be ready for my sign-off—no loose ends, no re-work.
We rely on a cloud-based EHR, so you should already be comfortable jumping between templates, shortcuts, and ICD-10 codes while staying HIPAA-compliant. Strong English grammar, medical terminology specific to musculoskeletal and neurological conditions, and quick keyboard skills are essential.
Deliverables
• Fully drafted follow-up, new-patient, and progress notes for every scheduled visit each day
• Accurate problem lists, medications, and orders entered directly into the chart
• Daily summary confirming all charts are closed or flagged for my quick review
