I'm looking for a professional who can create a comprehensive Insurance Application Form for me. The form will need to include a variety of question types and cater to a range of information.
The form should focus on client quoting specifics. The form should be digital. The form should capture detailed client-specific information.
I'd basically looking for the attached form to be functional. I'd like people to be able to answer questions in text in many spaces. I'd like the radio buttons to work. The two drop down menu's only contain 4-5 choices. I'd prefer the backgound it be in a light blue instead of red. I'd like all the fonts to be consistent. I think that's about it. Pretty basic. Brad
