Digital Reunion Invitation Design

I’m putting together a class reunion and need a casually styled digital invitation that classmates can open on their phones or screens. The artwork should feel friendly and fun, then feature a prominent QR code that directs guests to an online form. When they finish that form, I’d like their answers automatically emailed to me.

Here’s what must appear on the graphic itself:
• Date & time
• Location
• Cost and where to send payment
• Fields to remind respondents to give: first, last & maiden name, plus number of tickets being purchased

Deliverables
1. Final invitation file (PNG and PDF, high-resolution, ready to share by email, text, or social media).
2. QR code linked to a live form (Google Form, Typeform, or a simple web form—whichever you suggest) already containing the questions above and configured to route each submission straight to my inbox.
3. Brief hand-off note explaining how to update the form later if details change.
