Windows/mac Consultation Form Builder
Budget: $30 – $250 AUD
I need a straightforward Windows & Mac desktop application that recreates my current PDF consultation sheet, complete with our service and price list. My team should be able to open the fillable PDF, tick or type in the relevant services while they talk with the client, and then click once to:
• generate a clean PDF that mirrors the on-screen form
• email that PDF directly to the client from inside the app (using our preferred SMTP settings)
• store an identical copy locally or on a shared drive for our records
The on-screen layout must match the existing PDF so staff feel instantly familiar. No extra bells and whistles—speed and ease of use are critical. I’m happy to supply the original PDF and walk through the current manual workflow so you can map every field accurately.
Deliverables
• Windows & Mac executable and all source code
• Branded, fillable form UI aligned to the supplied PDF
• Automated PDF creation, email dispatch, and local save functions
• Brief user guide so new staff can get started in minutes
I’ll test by running the app on a standard Windows 11 machine, filling in a sample consultation, and confirming the emailed and saved copies arrive exactly as expected.
