Virtual Name Queue and Data tracking program

I am a teacher looking to use a simple windows computer program that I can leave open on my laptop during specific lessons which has the following functions (mock up diagram attached):

-Students who are wanting to conference with me can touch their name on my laptop screen (editable names for new year classes) so it appears in a virtual queue that we can all see on that same screen. Names move up as the top name is resolved (see picture mock up)
-Log date-stamped (DD/MM/YYYY) notes I can quickly jot down in a dialogue box for the name at the top of the queue which will then be automatically filed locally as a txt file under the same name so I can track my notes over time for that specific name in one txt file. This means writing a note for a name with no previous entries creates a new txt file that matches the name. Writing a note for a name with a txt file already created will add the new note in as a new line underneath the old one along with the date of entry. -Visually track who I have conferenced with and how many times with a number on their name. (see picture mock up)

Please give me a quote for how much something like this would cost to make. It doesn't have to look fancy, just be quick and easy to use in a busy classroom, be easy to tell at a glance which names I have not conferenced with enough, and have all of the entries compiled together into one txt file per unique name. I would also like to see the queue and the list of name at the same time on the same screen (as roughly pictured in the mock up). The ability to rename the name boxes for a different class the next year will be useful, even if it means I have to manually cut and paste the txt file notes elsewhere before changing the names for a new year and class.
