Permit to work database system

We have a paper based permit to work system with forms available in Word format. We want to convert this to a computer based system that we can easily amend in the future as required. We will require a general permit to work for all jobs and then some jobs will require additional permits such as hot work, work at height, confined space entry etc.
Ideally we would like to use an open source database such as PostgreSQL unless advised otherwise.
