Survey Database Data Entry

I have fewer than ten completed survey questionnaires that must be transferred into my existing database with perfect accuracy. Each response includes the usual text fields (names, emails, multiple-choice answers and open comments), and I need every detail recorded exactly as it appears on the forms, checked for spelling inconsistencies and obvious duplicates before upload.

You may work directly inside my cloud-hosted table (MySQL via phpMyAdmin) or prepare a clean CSV/Excel file for import—whichever is faster for you—so long as the final result drops seamlessly into the live database without encoding issues. If you notice blank or ambiguous answers while keying, flag them so I can follow up with the respondent rather than guessing.

Deliverables
• All 1–10 surveys entered as individual records
• Internal consistency review complete (no missing mandatory fields, no accidental extra spaces)
• A brief log noting any flagged anomalies or clarifications required
