Form for bank reconcialation and approval

We need to design a simple system where the branches can make request for refund from customers and they need to filled a form which is then shared with the accountant to process, and the account need to filled a form (to make sure all fields are inputted correctly) as well. The branches should not be able to modify once the accountant fill his part. Need similar for payment received by bank transfer, where the branches make a request to accoutant to confirm payment has been received.
