FileMaker Questionnaire Solution
Budget: $250 – $750 AUD
I’m looking for a lean Claris FileMaker file that lets me launch three questionnaire types—Customer Survey, New Client Intake, and Employee Feedback—while relying on one master list of static questions. Each time a user starts any of those questionnaires, they should simply create a new “response” record tied to that question set, so the database grows with every run but the questions themselves never change.
All data must stay in a single FileMaker database, neatly related so I can view, search, and report on answers by questionnaire type, date, or respondent. No conditional branching is required; every respondent sees the same questions in the same order.
What I need from you:
• A clean relational structure (Questions, Questionnaires, Responses, and Answers) with appropriate layouts for entering and reviewing data.
• Simple navigation buttons or a launcher screen so staff can pick the questionnaire type, start a new session, and then save/print/email a summary once finished.
• Basic scripts to automate record creation, prevent duplicate submissions in the same session, and lock responses after completion.
• A brief walkthrough or screen-recorded video so I can maintain and extend the file myself.
