Expert Database/Model Designer

I am seeking a VERY experienced data modeller (ER/Grapg or both) to design a database model for a quality maintenance system. The present data model was built in a hurry and is poorly designed. It also currently supports a single organistion type. There are currently two data models. One model supports the data capture for inspections while the other captures reporting at the organisation level. The model needs to be able to support capture of inspections/reporting dta for multiple organisation types during inspections at the different levels of the different inspections. Below is a summary:

Use case:
I would like to conduct inspections of a ward in my hospitals by asking the staff a number of questions. These questions are already captured in the question bank. I would also like to do the survey/inspections at the level of the hospital and also division (highest level). The model should also support inspections for different sectors (e.g. care homes, pharmacies etc).

I want to be able to generate reports for the organisation. I will also be looking to eventually develop data warehouse model from this.

I don't expect this to be complex to deliver for a seasoned data modeller. I will provide more details and work with the developer through the requirements. I expect to engage for a period of one month.. possbly beyond.
