Database for organiser

We would like to develop a database for a web-app that allows subscribed Users to store information within ready made forms and categories. For example, if a User logs in to their account and selects the 'TRAVEL' category, this will open a list of Sub-Categories (in the Travel genre). One such sub-category would be called 'Passports' and this would be the logical place to save your Passport details and attach one or more images of your passport. The User can Login to their account at any time to quickly find their saved passport details. The User could also Add additional 'Passport' entries and view a full list of ALL the entries that they have saved in 'Passports'. These entries can be Viewed as a list or opened individually to Edit or Delete them. The same applies to any forms/entries in any of the 40 or so sub-categories.
Additional User facing requirements for this database are:
a/ when a user logs in to their account, they should have the ability to click a button to 'download ALL their stored information/forms into a single csv file'.
b/ and also the ability to click a button to 'download ALL their attached files/images in to a single zipped file'.
c/ a Number Counter should also be displayed to show the User the exact number of entries they currently have stored within every sub-category in their account.
d/ an advanced search field to allow Users to search for any information they have stored via keywords.
Additional Internal Admin facing requirements:
a/ an up to date list of ALL Users and their date of registration to the Service.
b/ an up to date list of the number of entries ALL User's have stored in ALL of the sub-categories in their account.
c/ an up to date list of last login dates for ALL Users and the names of the sub-categories they clicked on during their last visit.
d/ the ability to download a copy of the entire DB so that we can back it up and keep it safe.
