Ignition Dynamic Form Database Build

I need to turn a paper-style project request form into a fully database-driven workflow inside Ignition (or another stack that can be tied back into Ignition). A user will open a “Request Project” screen, pick anywhere from one to all of our task options, and the form must instantly reshape itself—dynamic fields appear, conditional logic keeps things tidy, and common information is pre-filled when we already know it.

Every request also has to accept file uploads—images, documents, and videos—and those files must stay linked to the record for future audits. After the first submission, a project owner or system admin should be able to reopen the record, add completion notes, or change any detail while the system quietly keeps a timestamped revision history.

What I need from you:

• A clean, normalized database schema that captures the request, the checklist of tasks, every attachment, and a revision log.
• An Ignition Perspective/Vision view (or an equivalent web front end) that delivers the dynamic, conditional, pre-filled form experience.
• CRUD screens so admins can review, edit, and export records.
• All files and data must remain available for audit retrieval years down the road.
• A short deployment guide so I can move the solution between test and production gateways.

Acceptance criteria:
1. I can submit a request, watch the form adapt in real time, and see the data land in the database.
2. I can upload an image, a PDF, and a video, then download them later.
3. I can edit a record as an admin and see both the old and new versions.
4. I can export a single record—including its attachments—for auditors.
